Have you recently been told that you need to have a hysterectomy? A hysterectomy is the surgical removal of the uterus and may be performed for many different reasons. When you have this procedure, it can significantly impact the strength and tension in your abdominal and pelvic floor muscles. Pelvic floor physical therapy can be a great tool to combat the effects of a hysterectomy both before and after the procedure.
The Role of the Pelvic Floor
The pelvic floor consists of a group of muscles, ligaments and organs including the rectum, bladder and uterus. The pelvic floor muscles act as a hammock to support these organs and maintain normal functioning. When the uterus—and sometimes the ovaries—are removed during a hysterectomy, it can disrupt several areas of the pelvis. This could potentially lead to urinary incontinence, pelvic organ prolapse, pelvic pain, and core weakness.
Addressing your pelvic health before and after surgery has been shown to improve surgical outcomes and prevent complications that may arise down the road. Your pelvic floor therapist will help you feel more confident, stronger and better prepared for your journey ahead.
Preparing for a Hysterectomy with Pelvic Floor Physical Therapy
You might be wondering why it would be important to work with a pelvic floor therapist before the surgery. During the sessions before your surgery your pelvic PT might focus on the following:
Performing a Baseline Assessment: It’s important to know how strong your muscles are and if there is any muscle tension or pain that needs to be addressed before the surgery.
Improving Awareness and Strengthening: Understanding how to properly activate (and relax!) your pelvic floor and deep core helps tremendously when we begin strengthening again after surgery. Your PT may begin a strengthening program with you before the surgery so you start with as much strength as possible.
Education on Post-Surgical Care: Your PT will provide tips for moving safely and avoiding unnecessary strain during recovery.
Recovering After a Hysterectomy
Once you’ve had surgery, we will focus on recovery. Postoperative pelvic floor physical therapy will consist of rebuilding strength, restoring function, and addressing any symptoms you might have. This may include:
Pelvic Floor and Deep Core Exercises: This will help strengthen and stabilize your trunk and pelvis. When you have any type of pelvic or abdominal surgery, there is a recovery period that requires rest and reduced physical activity which can weaken these muscles. Building strength in this area will make your recovery much easier.
Addressing Scar Tissue: If you have any abdominal or pelvic scars, your therapist can work on mobilizing this tissue to prevent tightness or discomfort.
Managing Symptoms: If you experience issues like incontinence, urgency, or pelvic pain, your therapist will create a plan to help you manage and recover from these symptoms.
Gradual Return to Activity: Your recovery plan will include guidance on how to safely return to your favorite activities without overdoing it.
